Smile, in the Name of the Queen is awarded for using either Cherry Blossom Twigs or the Cherry Blossom Branch memento to festoon other players with cherry blossom wreaths. These can be found in Jester's Festival Boxes as rewards for completing the daily quests during the Jester's Festival. A single use of either item can affect as many as 5 players at once if they're all within range, and you can be credited multiple times for getting the same players, so one good strategy is to pick a popular wayshrine or other place where there are large crowds and use the blossoms there. You can receive this achievement with as few as 4 throws.
